Summer camp is a big Scouting step. It's a time for fun, growth, newfound freedom, responsibility and learning - for both Scouts and parents.

Required Forms for Camp
All Scouts and Adults that attend camp must submit the forms listed below

Annual Health and Medical Record
All parts (A, B, and C) must be completed.
Note 1: Part B1 requires that you attach a photocopy of both sides of the medical insurance card.
Note 2: Part C is to be filled out by a qualified medical professional and requires a physical exam. -
Routine Drug Administration Record
List all prescription medication to be taken at camp.
If none, do not submit form. -
Pre-Event Medical Screening
COVID-19 medical screening. -
Firearms Permission Form.
This form is optional. If not turned in, the person will not be allowed to shoot at any of the ranges (rifle, shotgun, archery).
